Permanent Supportive Housing

Considered a solution to homelessness, permanent supportive housing allows individuals to live independently while having access to cost effective and humane on-site services to support a resident through relapse, loss of income, serious physical health and general mental health issues and daily living skills. With such supports, residents are able to increase their housing stability, reduce reliance on expensive public services and improve their overall health and well-being. Recognizing the need for PSH, Native American Connections brought their history of serving vulnerable people to the development and management of four PSH projects to serve low income and chronically homeless individuals living in metro-Phoenix.

Applicants are taken from the coordinated waitlist. The waitlist is "weighted" based on chronicity and acuity. All applicants must provide written documentation of homelessness and permanent disability, as well as being assessed prior to being placed on the waitlist. Other occupancy restrictions may apply. NAC Interest list update

Once placed on Interest list - applicant must complete and send an update form at least every 120 days or they will be dropped from the Interest List.

Section 8 and VASH vouchers are welcome at all apartment communities. Vouchers are available through the City of Phoenix Housing Department and Department of Veterans Affairs.

The community includes 10 units for homeless adults and 8 units are reserved for individuals with serious mental illness; 15 units with Section 8 vouchers for chronically homeless individuals.
