Native American Connections offers Telehealth for all Outpatient Services. Call 602-424-2060 to schedule an appointment. New clients can complete an assessment over the phone.

Beginning June 1st, NAC is offering limited in-person services.  All clients are required to wear a face covering and practice social distancing.  A mask will be provided if you do not have one.  Other safety protocols may be in place including temperature checks and completion of a COVID-19 Screening Questionnaire.

The first step in connecting with any Native American Connections behavioral health service is to complete an intake and assessment.  This helps our staff determine which programs and services would be beneficial to you. ( Referral Intake form )
